Heard This at a James Gregory performance:

Hey! Me and My wife were in Miami about to get on one of them 2-engine prop jobs to hop over to the Bahamas.

The pilot comes over to me an asks me and my wife "How much y'all weigh?"

I say WHAT!!!" "Why you need to know how much we weigh?"

He says "So I'll know how much gas to put in."

I say "$#!+ !! Fill-it-up!!" "I'd hate to know I died in a plane crash cuz some fat lady lied about how much she weighed."

Funny joke. But seriously if it was necessary to fly a bunch of people who might be sensitive about their weight better to ask "How much do you AND YOUR BAGS weigh? I don't care which is which but I need an accurate TOTAL NUMBER to keep us safe." If weight and balance is an issue I guess the bags could always be weighed after the people board.

Airlines weigh the checked baggage but not the carry-ons or the passengers. So how do commercial airlines determine total weight? Are averages per person accurate when you have hundreds of people?

Maybe not accurate, but not terribly unsafe if you assume high enough averages. Of course, the airlines have a financial incentive to squeeze as much weight as they can onto every flight.
