does the FAA automatically do a medical background check or is it only if you check yes?

When you sign the MedExpress form, regardless of what you put down on the form, you've given your consent for the FAA to inspect your medical history and records. If there's anything out there that would be disqualifying, you have to assume the FAA will find it, and revoke any medical certificate you may have been issued. If that happens, you lose not just your medical certificate, but the ability to exercise Sport Pilot privileges.

Thus, if you know (or have reason to know) of any potentially disqualifying condition, you're best off not even starting the application process, and just proceeding under Sport Pilot rules and restrictions.

Kinda what I figured. I’m think I’m just going to play it safe and stick with sport pilot. As it will do most of what my intentions will be

That's a good idea. Many of us, new pilots and old pilots, have made that decision.
